Nurse-Led Digital Self-Management Intervention pada Pasien Penyakit Kronis: Scoping Review terhadap Komponen Intervensi, Outcome Klinis, dan Implikasi Keperawatan Medikal Bedah

Penyakit kronis masih menjadi tantangan besar dalam pelayanan kesehatan karena memerlukan perawatan jangka panjang, kepatuhan terapi, perubahan perilaku, dan pemantauan berkelanjutan di luar rumah sakit. Dalam keperawatan medikal bedah, pasien diabetes melitus, hipertensi, penyakit jantung, PPOK, stroke, kanker, dan multimorbiditas sering mengalami hambatan mempertahankan self-management setelah pulang dari fasilitas kesehatan. Digital health memberi peluang bagi perawat untuk memperluas edukasi, monitoring, konseling, dan koordinasi perawatan melalui intervensi digital yang dipimpin perawat. Artikel ini bertujuan memetakan komponen nurse-led digital self-management intervention pada pasien penyakit kronis, mengidentifikasi outcome klinis dan nonklinis, serta merumuskan implikasinya bagi keperawatan medikal bedah. Review disusun dengan pendekatan scoping review berdasarkan Joanna Briggs Institute dan PRISMA-ScR, berfokus pada literatur internasional tahun 2020–2026. Hasil sintesis menunjukkan lima komponen utama dan berbagai outcome positif, meskipun masih terdapat tantangan terkait platform, literasi digital, kesiapan perawat, keamanan data, dan integrasi layanan.
